Liverpool striker Mario Balotelli arrives for AC Milan medical

AC Milan have confirmed that Liverpool striker Mario Balotelli has arrived in the Italian capital to complete his medical ahead of a proposed season long-loan switch to the Serie A side.

The 25-year-old has fallen out of favour under Brendan Rodgers and is edging towards an Anfield exit after just one turbulent season on Merseyside.

"At 8.26 this morning, Mario Balotelli has reached the La Madonnina clinic for medical examinations provided for today," read a statement on Milan's official website.

The forward scored one Premier League goal last season and struggled to justify Liverpool's decision to pay Milan £16m for his services.

Such a poor strike rate has also cost the forward a place in the Italy squad and he will hope a return to the San Siro, where he scored 26 goals in 43 league appearances after ariving from Manchester City in January 2013, will reignite his career.

Balotelli has been warned by Milan CEO Adriano Galliani that the loan move represents a final opportunity to prove his worth.

"Balotelli convinced us to get him back," Galliani said. "He is aware that we are giving him his very last chance."
